Job Description:
Sanford Health is seeking a seasoned Senior Social Worker
specialized in Dialysis to join our dedicated team at our
state-of-the-art healthcare facilities. This role involves
providing critical psychosocial support, counseling, and resource
guidance tailored to the needs of patients undergoing dialysis
treatment. Our ideal candidate will help develop personalized
support plans that enhance patient compliance and improve their
overall quality of life. Assess the psychosocial status of patients
and their families to determine the types of mental health support
and resources needed. Collaborate with healthcare professionals to
integrate social services with patient care. Develop and implement
strategies to address psychological and social barriers to health
and wellness. Conduct regular follow-ups to evaluate effectiveness
of planned interventions. Advocate for patients' needs to ensure
access to required resources. Maintain comprehensive documentation
of patient interactions, conditions, and progress. Strong empathy
and communication skills Ability to handle high-stress situations
Proficiency in creating and implementing tailored care plans Deep
understanding of the impact of chronic illness on individuals and
families Competence in case management software and patient
databases Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Licensed Clinical Social Worker (LCSW) Certification
in Nephrology Social Work (CNSW) preferred
