Department Details This is a great opportunity to work alongside a

care. Job Summary The Vascular Surgery Clinic Triage Nurse is a
registered nurse responsible for providing initial assessment and
triage for patients seeking care in a busy vascular surgery clinic.
This nurse will assess patient needs, determine the urgency of
their condition, and facilitate appropriate follow-up care. The
triage nurse collaborates with surgeons, clinicians, and other
healthcare professionals to ensure timely and accurate care for
patients with vascular conditions. The clinic offers a flexible
Monday through Friday schedule, with no weekends or holidays
required. Key Responsibilities: Patient Assessment and Triage:
Conduct initial assessments of patients’ medical history, symptoms,
and vascular concerns. Prioritize patient concerns and assess the
urgency of care required. Guide patients on the appropriate steps
for care, whether through surgery, diagnostic testing, or follow-up
consultations. Clinical Support: Assist with the scheduling of
appointments, diagnostic tests, and follow-up visits based on
triage assessments. Provide information to patients regarding
vascular conditions, treatment options, and what to expect during
their care. Administer basic clinical procedures, such as taking
vital signs, collecting patient history, and assisting in wound
care as directed by the physician. Collaboration with Medical Team:
Communicate effectively with vascular surgeons, advanced practice
practitioners, and other healthcare team members about patient
concerns, assessments, and triage decisions. Relay relevant
clinical information to the medical team for timely diagnosis and
intervention. Participate in patient care planning, follow-up
discussions, and ongoing treatment protocols. Patient Education and
Advocacy: Educate patients about vascular health, preventive
measures, and pre- and post-operative care. Address patient
inquiries and provide clear explanations regarding vascular
conditions, treatment plans, and lifestyle changes. Act as a
patient advocate, ensuring that patient concerns are addressed in a
timely and compassionate manner. Documentation and Compliance:
Maintain accurate, up-to-date, and complete patient records in
accordance with clinic policies and regulatory requirements. Ensure
compliance with healthcare laws and regulations regarding patient
privacy and confidentiality (e.g., HIPAA). Document triage
assessments, interventions, and outcomes in the electronic health
record (EHR). Quality Improvement: Participate in continuous
quality improvement (CQI) efforts related to patient care and
clinic efficiency. Identify areas for improvement in patient triage
processes and recommend solutions to enhance patient care delivery.
Stay current with the latest vascular surgery techniques,
practices, and guidelines. Other Duties: Assist with patient flow
and manage clinic workload during high-demand periods. Provide
backup support in other clinical areas as needed. Skills and
Competencies: Strong clinical assessment and triage skills.
Excellent communication and interpersonal skills, with the ability
to interact compassionately with patients. Ability to prioritize
tasks in a fast-paced environment. Proficient in electronic health
record (EHR) systems. Knowledge of vascular anatomy, surgical
procedures, and treatment options is a plus. Ability to work
effectively as part of a multidisciplinary healthcare team. Guides
the patient to the appropriate level of care; schedules patient
appointments over the phone with appropriate healthcare
providers/facilities/physicians. Documents information provided to
patients during triage in health record systems; records patients
health conditions for follow-up patient care. Maintains and
develops triage protocols; ensures patients' healthcare information
is updated on time. Role does answer inquiries through multiple
variations of technology, depending on department. Educate patients
based on calls that come in. Qualifications Graduate from an
accredited nursing program preferred, including, but not limited
to, American Association of Colleges of Nursing (AACN),
Accreditation Commission for Education in Nursing (ACEN), and
National League for Nursing Commission for Nursing Education
Accreditation (NLN CNEA). Bachelor’s Degree in nursing preferred.
One year of healthcare experience preferred. Currently holds an
unencumbered RN license with the State Board of Nursing where the
practice of nursing is occurring and/or possess multistate
licensure if in a Nurse Licensure Compact (NLC) state. Nurses
performing nursing practice over the telephone require licensure in
the states where the patients being served reside. Within first 30
days of employment, all requirements to obtain license needed for
role is completed. Obtains and subsequently maintains required
department specific competencies and certifications. Obtains and
