Job Description:
Monday - Friday8:00am - 4:30pmNo weekends, no holidays! Scope
Work closely with clients, family members, and other home health
and therapy staff. Provide work direction, supervision and
delegation to the Home Health Occupational Therapist Assistants and
home health aides (if applicable). Responsible for organizing,
developing, and directing occupational therapy services in
accordance with the standards and practice of occupational therapy,
state and federal regulations, and Ethos' policies and procedures.

Experience Required Must complete and pass a state required
background study. Graduate of an accredited school of occupational
therapy with a current license and/or registration to practice
occupational therapy in Minnesota and North Dakota. Must obtain CPR
certification within 6 months from date of hire. Must have a valid
driver's license and use of an insured personal vehicle. Ability to
incorporate all aspects of occupational therapy in client care.
Ability to instruct and supervise personnel to assure adequate and
competent client care. Abide by the HIPAA (Health Insurance
Portability and Accountability Act) privacy policy as administered
by Ethos. Must be organized and able to complete required tasks on
schedule. Capacity to grasp new theories and principles and
implement them in practice. Computer skills and solid management of
documentation processes. Able to physically perform the duties
required by the position. Required to have the ability to read,
write and speak English well enough to communicate with staff,
patients, families and facilities. Key Responsibilities Direct Care
ActivitiesEvaluate clients; obtain and interpret data necessary for
treatment from medical records, observation of client in natural
setting, interviews, and administration of standardized tests and
measurements. Develop treatment plans and programs, including short
and long-term anticipated functional outcomes, and administer
occupational therapy treatment. Obtain approval for services from
physicians or designees; and provide treatments per physician's
orders. Provide quality care that meets client's goals and needs by
performing periodic client re-evaluations to determine
effectiveness of treatments; then adjusting treatment plan and
program appropriately to reach functional outcome. Work closely
with RN case managers and intake personnel for client intake, care
planning, and therapy discharge planning of clients. Maintain same
day admission philosophy and communicate when this is unachievable
and coordinate with RN case managers when admission can occur.
Communicate to case manager and Regional Clinical Manager when
admission cannot occur within 48 hours. Client Records and
BillingProvide accurate and timely documentation of services
rendered utilizing specific home health software. Demonstrate
understanding and knowledge of all payer sources. Review and sign
documentation of the Physical Therapist Assistant as required;
complete appropriate visits for COTA supervision if applicable.
Complete and/or check accuracy of completion and timely submission
of billing information and appropriately account for all services
rendered. Keep schedule on EMR as accurate and up to date as
possible. CommunicationDemonstrate exceptional verbal and written
communication skills for purpose of work tasks and patient care.
Able to clearly communicate with patients, families, staff and
others to meet job requirements. Modify and adjust communications
as needed to reach common understanding. Must have a cellular phone
available and be know how to use it. Attend IDT meetings as
scheduled. Follow-up ProgramsOptimize successful discharge from
therapy for patients with highest function and least amount of risk
possible allowed per entitlement. Educate client and
family/caregivers about transition to restorative, wellness or
nursing programs. Educate client in appropriate home exercise
program or other health management program. Recommend and train in
use of appropriate assistive equipment and environmental
adaptations to maximize function. Professionally connect with
physicians, outside vendors and other healthcare professionals of
services when appropriate to ensure continuum of care. Leadership
and Organizational SkillsShow exemplary qualities in attitude,
effort and work performance. Work within parameters of budget and
resources available. Evaluate outcomes of therapy interventions.
Complete additional duties, projects, in-services, promotional
events and program development as assigned by Regional Clinical
Manager. Professional GrowthMaintain professional and clinical
knowledge by attending facility in-services and workshops/training
on topics that provide depth to Ethos programming and professional
development. Share and collaborate on knowledge acquired with team
members for patient success. Demonstrate use of evidenced-based
practice. Participate in clinical education opportunities for
students of the profession. SafetyFollow safety procedures for work
environment and promote client safety while performing direct care.
