We are seeking a passionate Registered Nurse witha desire to learn delivery of care to pediatrics in a home health setting, or recentexperiences in pediatrics, to join this reputable organization dedicated to providing high-quality home health care services.

As a Pediatric Nurse, you will have the chance to work closely with children and their families, providing specialized care and support.

Responsibilities/Essential Job Functions:

• Perform initial visit and re-evaluate patient's needs and progress on a regular basis.
• Initiate plan of care under doctor's orders.
• Observe, assess, document symptoms, and monitor reactions and progress.
• Provide education and support to patients and caregivers on techniques for in home health care, disease process, medications, plan of care and individualized treatment plans.
• Coordinate patient services and supervise LPN's and HHA's.
• Notify physician and DON or Administrator of change in patient's condition.
• Discharge patient from skilled nursing services when criteria has been met.
• Case conference clinicians providing care to ensure coordination of care.
• Update records and maintain compliance with agency policy and procedures.
• Adhere to state regulations.
  Qualifications:
  
  - Registered Nurse (RN) with a current license and one year of RN experience.
  
  - Experience in pediatrics, trach, g-tube and central line preferred or is willing to learn.
  
  - Work positively and favorably with clients, families and staff.
